HOMABAY MUNICIPALITY 

The County Government of Homabay invites bids from interested and eligible bidders for the following Tender

Interested bidders should access the documents through the following websites: www.homabay.go.ke/www.suppier.treasury.go.ke/ www.tenders.go.ke

MANDATORY REQUIREMENTS.

1. A copy of business registration certificate/certificate of incorporation.
2. A copy of valid tax compliance certificate.
3. Registration with the National Construction Authority in the category indicated in the tender document
4. Bid bond of the sum stated in the tender document from a reputable financing institution or an insurance firm (for the citizen contractors) and dully filled, signed and stamped tender securing declaration form for Special Group
5. g, Duly filled, signed and stamped form of tender prepared on supplier’s letterhead clearly showing the Tenderer’s complete name and business address
6. Confidential business questionnaire duly filled and signed.
7. A copy of valid single business permit (S.B.P)
8. A copy of CR12 for limited company and for Sole proprietor S Partnership companies to provide copies of directors I.D)
9. Attach relevant AGPO Certificates for the Youth and Women- for PWD Attach Certificate of Registration with National Council for People Living with Disabilities.
10. Dully filled, signed and stamped Certificate of Independent Tender Determination Form
11. Dully filled, signed and stamped Self Declaration Form SD2
12. Dully filled, signed and stamped Declaration and Commitment to the code of ethics Form
13. Written declaration letter signed and stamped by an attorney/commissioner for oaths that the firm/person/tenderer is not debarred from participating in procurement proceedings
14. Written declaration letter signed and stamped by an attorney/commissioner for oaths that the firm/person/tenderer is not guilty of violation of fair employment laws and practices
15. Written Power of Attorney for the person(s) duly authorized to bind the tenderer certified by commissioner of oaths.
16. Tender document MUST be sequentially Paginated/serialized/ Numbered on each page including all the attachments.

NOTE:

1. Canvasing is highly discouraged and may lead to automatic disqualification.
2. Failure to comply with any of these requirements will lead to disqualification of the tender.

Prices quoted should be net, inclusive of all taxes and delivery costs expressed in Kenya shillings and shall remain valid for a period (119) days from the closing date of the tender.

Complete tender document should be submitted through the supplier portal: www.supplier.treasury.go.ke  on or before 15^th  March, 2022. Original bid security must be delivered to Homa Bay County Supply Chain Management Office during the normal office hours before the tender dosing date.
