COUNTY GOVERNMENT OF HOMA BAY TENDER SEPTEMBER 2022 

DEPARTMENT OF HEALTH SERVICES

The County Government of Homa bay through the Department of Health services invites bids from interested and eligible firms for the supply and delivery of CT Scan Machine to the Homa bay County Teaching and Referral Hospital.

MANDATORY REQUIREMENTS

1. A copy of business registration certificate/certificate of incorporation.
2. A copy of valid tax compliance certificate.
3. Duly filled, stamped and signed Manufacturer’s Authorization form prepared on the Manufacturer’s letter head.
4. A copy of current CR12 for limited company and for Sole proprietor & Partnership companies to provide copies of directors I.D]
5. Duly filled, signed and stamped Certificate of Independent Tender Determination Form
6. Written Declaration letter signed and stamped by an attorney/ commissioner of oaths that the firm/ person/tenderer is not debarred from participating in procurement proceedings [form SD1)
7. Duly filled, signed and stamped Self Declaration Form SD2
8. A copy of valid Single business permit
9. Duly filled, signed and stamped Declaration and Commitment to the code of ethics Form
10. The Written confirmation of authorization to sign on behalf of the Tenderer shall consist of: written Power of Attorney for the person[s) duly authorized to bind the tenderer certified by commissioner of oaths.
11. Tender document MUST be sequentially Paginated/serialized/ numbered on each page including ail the attachments.
12. All tenderers to provide Ksh. 900,000 tender security from a financial institution regulated by Central Bank of Kenya [CBK].
13. Duly filled, signed and stamped form of tender prepared on supplier’s letterhead clearly showing the Tenderer’s complete name and business address.

Failure to comply with any of these requirements will lead to automatic disqualification of the tender.
