TENDER FOR CONSTRUCTION OF COMPREHENSIVE OUTPATIENT BLOCK 2022

COUNTY GOVERNMENT OF HOMA BAY TENDER SEPTEMBER 2022 

DEPARTMENT OF HEALTH SERVICES

The County Government of Homa bay through the Department of Health Services invites bids from interested and eligible firms for the construction of comprehensive outpatient block at the Homa bay County Teaching and Referral Hospital.

MANDATORY REQUIREMENTS

1. Copy of certificate of incorporation/Registration
2. Copy of a valid tax compliance certificate
3. Copy of relevant Valid National Construction Authority Registration Certificate (NCA Category 6 and above in the Category of Building Works)
4. Must submit a copy of relevant current National Construction Authority Practicing License ((NCA Category 6 and above)
5. A copy of CR12 for limited company and for Sole proprietor & Partnership companies to provide copies of directors I.D)
6. Duly filled, signed and stamped form of tender prepared on supplier’s letter head clearly showing the Tenderer’s complete name and business address
7. A valid Single business permit
8. Dully filled, signed and stamped Confidential Business Questionnaire
9. Dully filled, signed and stamped Certificate of Independent Tender Determination Form
10. Dully filled, signed and stamped Self Declaration FormSD2
11. Dully filled, signed and stamped Declaration and Commitment to the code of ethics Form
12. Written declaration letter signed and stamped by an attorney/commissioner of oaths that the firm/person/tenderer is not debarred from participating in procurement proceedings
13. Tender document MUST be sequentially Paginated/serialized/ numbered on each page including all the attachments.
14. All tenderers to provide Ksh. 500,000 tender security from a financial institution regulated by Central Bank of Kenya (CBK). Original bid bond should be deposited and registered with procurement secretariat at the office of the County director of procurement while its copy to be uploaded with the tender document.
15. Written power of attorney for the person(s) duly authorized to bind the tenderer certified by commissioner of oaths

Failure to comply with any of these requirements will lead to automatic disqualification of the tender.
