MANDATORY REQUIREMENTS.

  1. A copy of business registration certificate/certificate of incorporation.
  2. A copy of valid tax compliance certificate.
  3. Submit copy of valid tender security as indicated in the Tender Data Sheet.
  4. Copy of relevant Valid National Construction Authority Registration Certificate (NCA Category 5 and above in Building Works and Mechanical works where applicable
  5. Duly filled. signed and stamped form offender prepared on suppliers letterhead clearly showing the Tenderer’s complete name and business address
  6. Must submit a copy of relevant current National Construction Authority Practicing License (NCA Category indicated in the tender document where applicable in Building Works)
  7. All tenderers to provide tender security In the form of a Demand Bank guarantee from a financial institution regulated by Central Bank of Kenya (CBK).
  8. Confidential business questionnaire duly filled and signed.
  9. A copy of valid single business permit (S.B.P)
  10. A copy of CR12 for limited company and for Sole proprietor & Partnership companies to provide copies of directors I.D)
  11. Dully filled. signed and stamped Certificate of Independent Tender Determination Form
  12. Dully filled. signed and stamped Self Declaration Form SD2
  13. Dully filled. signed and stamped Declaration and Commitment to the code of ethics Form
  14. Written declaration letter signed and stamped by the tenderer that the firm/person/tenderer is not debarred from participating in procurement proceedings
  15. Written declaration letter signed and stamped by the tenderer that the firm/person/tenderer is not guilty of violation of fair employment laws and practices
  16. Written Power of Attorney for the person(s) duly authorized to bind the tenderer certified by commissioner of oaths.
  17. Tender document MUST be sequentially Paginated/serlalized/ Numbered on each page including all the attachments.

NOTE:

  1. i)Canvasing is highly discouraged and may lead to automatic disqualification.
  2. ii)Failure to comply with any of these requirements will lead to disqualification of the tender.

Prices quoted should be net. inclusive of all taxes and delivery costs expressed in Kenya shillings and shall remain valid for a period (119) days from the closing date of the tender.

Complete tender document should be submitted through the supplier portal: vyww.supplier.treasury.go.ke on the close date of 30th  March. 2022. Original bid security must be delivered to Homa Bay County Supply Chain Management Office during the normal office hours before the tender closing date.
